Team Registration for the 2012-2013 Competition

This page provides all the forms you need to get your team(s) registered for the 2012-2013 San Diego Regional Science Olympiad. It's as easy as 1-2-3. You may bring the completed forms to the first coaches' meeting or mail them to the address at bottom of this page.

  1. First Team Registration Form
    A team has a maximum of 15 students. (There is no minimum number of students for a team.) For your first team, you are required to register and pay dues to the Regional organization ($40), the State ($50) and to the National Science Olympiad ($60) in order to participate in the Regional (San Diego County) Competition. Purchase Orders are NOT accepted and your check must be received before your team is registered. Only AFTER you have registered will you get a Rules Manual. You will get one manual per team.

    Important: This form and full payment is due no later than December 1, 2012.

  3. Additional Teams Registration Form
    Use this form to register your additional teams. Each additional team will be registered for $100 (National + Regional fee). You may also register additional teams at a later date, but all teams must be registered before December 1, 2012. Each team, including the first team, must have its own team name.
    Important: This form and full payment is due no later than December 1, 2012.
  4. Guarantee of Insurance Form
    This is the Guarantee of Insurance form for all schools that are NOT in the Poway Unified School District.
    This form to be completed in its entirety and returned by November 14, 2012. If any of your team members plan to attend the Workshop at Bernardo Heights Middle School on November 17, 2012, you may mail this form ahead of time or bring it, completed, with you to the Workshop Important: This form is due by November 14, 2012.
  5. PUSD Guarantee of Insurance Form
    For Poway Unified Schools Only. This form to be completed in its entirety and returned by November 14, 2012. If any of your team members plan to attend the Workshop at Bernardo Heights Middle School on November 17, 2012, you may mail this form ahead of time or bring it, completed, with you to the Workshop.

  6. Permission Slips
    Please check with your school to see if you need to have field trip permission slips for your students to attend the San Diego Science Olympiad Workshops on November 17, 2012, the Division B Regional Science Olympiad Competition on February 2, 2013 and/or the Division C Regional Science Olympiad Competition on February 17, 2013.

    In some states permission slips are required for all students travelling to any event in any vehicle other than an authorized school bus.  The purpose of these slips is to protect the drivers of private cars from litigation in the event of an accident.  Some permission slips note emergency numbers for each child and/or stated permission for necessary medical treatment.  Without the stated permission for necessary medical treatment, some Michigan hospitals have been unwilling to start medical care in the absence of parents or guardians.  You might want to consider this.  In California, we never assume that everything will proceed entirely without incident.

Volunteer Requirements per Team: Each TEAM MUST supply at least one adult volunteer.  The volunteers will report to the Registration Desk before 7:30 am on either February 2 or February 17, 2013.  They must be prepared to help the event to which they are assigned until at least 1:00 PM.  If they are concerned that they will miss their children’s engineering events, they may volunteer to help at the Competition on the other day.
